Getting booked twice?

Last night's Burton V City game, one of the City youngsters went steaming into a Burton player, using his right leg to tackle from behind, then he brought his left leg round to kick the bloke, just to make sure he was fouled good and proper. Amazingly, the ref did not issue a card, but it got me thinking that both of those actions should have been a yellow, and combined a straight red. Have we even seen a ref brandish 2 yellows for a 2 phase foul. No nor me.

Carlos Tevez V 4X2s took off his shirt (Yellow) and went into the crowd (Yellow) should he have been sent off.

Should have been sent off twice 6 game ban
Must have borrowed Vincent KOMPANY’s immunity card that means he can never be sent off no matter how reckless he tackles

I sent a player off this season for deliberately handling the ball and then kicking the ball away when I blew my whistle for the initial offence. He said you can’t do that, I said I can 👍

Closest is booking for foul, followed by booking for dissent.
Like Ray Stewart after Paul Walsh's penalty-winning* dive at Anfield in 1986...
